In-Person Evaluation & Case Management

Includes 5 phases:

1. Virtual In-Take Meeting with parents (student participation is not required at this time) for introductions, lay out the narrative, review history, discuss goals and expectations.

2. In-Person Evaluation of student includes:
   a)     Fine motor skills
   b)    Gross motor skills
   c)     Motor coordination skills
   d)    Ocular motor skills
   e)    Dominance testing
   f)      Reflex integration
   g)     Social engagement
   h)    Sensory processing
   i)      Executive functioning

3. Report preparation and distribution

4. FCR Virtual Meeting (Findings, Conclusions and Recommendations)

5. Case Management and consultation with other team members
